Welcome to Andress High School! We extend a warm welcome to you and your son/daughter. We encourage active participation and assistance from parents so that students can experience a successful and positive high school career. This year the registration forms are on-line through our web site. You will need to click on the
| RETURNING STUDENTS | NEW STUDENTS TO EPISD |
These forms must be completed, signed and returned on registration day. Filling out the forms on-line does not mean that the student is registered. A PRINTED COPY OF ALL FORMS must be brought to school during registration. Without these signed forms, students will NOT be able to proceed with registration.
(1.) Delinquent Station Where we will determine if the students owe library fines, book fines or club money. The other stations will be numbered and the numbers will correspond to the numbers on the student registration routing form. All students must have the routing forms signed at each particular station. The last station will be the counseling center. Student schedules will be distributed in the counseling center after all appropriate signatures have been obtained on the routing form. At this time the students will receive a packet containing all the Andress procedures, rules and regulations. Students will have to sign for these packets before receiving their schedule.

Shows the dates and times that registration will occur. Registration will start in the LIBRARY.

To help students know who to ask for help or information. Each student is assigned a counselor by alphabet and an Assistant Principal by grade level.

To inform parents of important dates such as parent-teacher conferences, report card distributions and holidays.

To highlight the AB block schedule which has the meeting periods for "A" day and for "B" day.
(2.) BUS ROUTES/RULES AND APPLICATION
Lists the times and pick up points for students requiring transportation. The rules are enforced by the school because it is against the law to distract a bus driver. Students who require transportation must fill out the transportation application!!!

By law, each student must bring an updated immunization record. Students in major sports and band must see the trainers for information regarding physicals.
(5.) INTERNET ACCESS FORM
Students use the internet throughout the year for various classes and research. This form must be signed for students to be allowed access to the internet.
(6.) HOME LANGUAGE SURVEY ONLY FOR NEW STUDENTS TO EPISD
To comply with Texas Education mandates, each student must have a completed and signed Home Language survey form in their academic folder. Unless you are in ESL/LEP classes, please write English as the primary language.
(7.) PREVIOUS SCHOOL ATTENDED FOR NEW STUDENTS TO EPISD
To assist the registrar in requesting previous school records.
(8.) DISCIPLINE RECORD FOR NEW STUDENTS AND INCOMING 9TH GRADE STUDENTS
To provide previous discipline information.
(9.) INFORMATION FOR SPECIAL POPULATIONS
To determine scheduling placement for students with special needs.
(10.) AGRICULTURAL QUESTIONNAIRE
To determine if a student qualifies as a migrant student.
(11.) STUDENT ATTENDANCE / 90 PERCENT RULE
To acknowledge that students and parents are aware that attendance is required 90 percent of the instructional time and loss of credit may be a consequence of non-attendance.
(12.) CRISIS CONTACT INFORMATION
To provide student contact information in case of emergency.
(13.) LUNCH APPLICATION
Federal funds are generated from the number of students who fill out and return these forms. These funds supplement the general school budget and allow for the improvement of instructional programs and equipment. Also, students who qualify for the programs are entitled to fee waivers when taking Advanced Placement exams. Colleges also use the information when working with financial aid. Regardless of income, all students must turn in a completed lunch application form.
2009-2010 Dress Code
To provide students and parents information on acceptable school attire thus avoiding an In-School Suspension for inappropriate clothing.
Attendance Rules
To highlight all STATE mandates tied to attendance and to outline the EPISD rules and procedures on excused and unexcused absences as well as tardies.
Discipline Procedures
To outline the Standards of Behavior required at Andress High. The consequences for inappropriate behavior are included.
Textbook Responsibilities
Explains important information about the care of State Textbooks.
The Following Forms are On-Line and will need to be filled out, signed and turned in during registration.
The Andress High School Library will remain open from 5p.m. to 6:30 p.m. the week of July should you need computer assistance with the forms.

STUDENT REGISTRATION ROUTING FORM
All students will use this form during the registration process. Students will not receive a class schedule until all signatures have been obtained. The form will be collected by the counselor when the student receives his/her schedule.
(14.) STUDENT ENROLLMENT FORM
Fill out the information on this form making sure that home phone numbers, work numbers, cell numbers, address or other information is correct.
(15.) RESPONSIBLE PERSON VERIFICATION
This form is required by EPISD, Pupil Services Unit. It ties to the attendance policies of the school district and Texas State Law.
(16.) DISPLAY OF PERSONAL STUDENT INFORMATION
To give Andress High School and EPISD permission to release student names to the press for recognition and to colleges, universities and the armed forces for various post secondary programs.
(17.) REGISTRATION INFORMTAION FOR MILITARY FAMILIES
To make sure the military liaison has updated information on all military families.
